Back to topPatients who wish to participate must complete a patient consent form to be issued a Weill Cornell CONNECT activation code during their next visit. This code will enable you to login and create your own user ID and password. You can also download the patient consent form from the login page located at this URL: www.weillcornell.org/connect

Back to topAll communication between you and your healthcare provider and care team using Weill Cornell CONNECT is carried over a secure, encrypted connection. Encryption means the information is coded in such a way that no one can read it during transmission. This secure connection utilizes industry standard Secure Socket Layer (SSL) 128-bit encryption. Your health records are stored behind a firewall to prohibit unauthorized access. In addition, Weill Cornell CONNECT access requires an ID and password that you create yourself.
Back to topFrom the main login page, "Click on Forgot Password?". After entering your information, you will be prompted with the password question you entered when you first created your account. After correctly answering this question, you will be able to create a new password. If you need additional assistance, please contact your physician office.

After entering your information, if you have an external email address on file, your Weill Cornell CONNECT ID will be sent to that email address. If you need additional assistance, or do not have a valid email address on file, please contact your physician office.

Make sure you do not enter extra spaces in either the Weill Cornell CONNECT ID or password boxes and try again. Ask your doctor to correct any inaccurate information at your next visit, or send a message to the practice to request changes and updates to your health record. Your health information is reviewed and updated in your electronic medical record each visit.
Back to topYou will generally receive an answer within 1-3 business days. Please note that Weill Cornell CONNECT should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.
Back to topProxy access is when you access the medical record of another patient in Weill Cornell CONNECT. Examples may include a mother accessing her daughter’s chart, a husband accessing his wife’s chart, or a caregiver accessing the chart of his or her patient. The person accessing the chart is known as the proxy. Proxy access is only available for Pediatric patients up to the age of 11 to a parent or legal guardian. We currently are not giving proxy access for patients who are over 11 years of age.
Back to topNo, you should not ask questions regarding a family member from your Weill Cornell CONNECT account. Weill Cornell CONNECT offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.
Back to topNo, due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own Weill Cornell CONNECT account.
Back to topWe aim to protect your privacy and security of your information. While logged into Weill Cornell CONNECT, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of Weill Cornell CONNECT. We recommend that you log out of your Weill Cornell CONNECT session if you need to leave your computer for even a short period of time.

Back to topFor your security, your activation code expires after 45 days and is no longer valid after the first time you use it. If you still have problems, contact your physician office.
Back to topNo, your activation code is not your Weill Cornell CONNECT ID or password. You will use this code only once to log into Weill Cornell CONNECT for the first time. (The code will expire after you have used it or after 45 days). When you log into Weill Cornell CONNECT the first time, you will then be asked to create your own unique Weill Cornell CONNECT ID and password.
