Weill Cornell CONNECT offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With Weill Cornell CONNECT, you can use the Internet to:

• Request medical appointments.
• View your health summary from the Weill Cornell CONNECT electronic health record.
• View test results.
• Request prescription renewals.
• Access trusted health information resources.
• Communicate electronically and securely with your medical care team.

Is there a fee to use Weill Cornell CONNECT?

Weill Cornell CONNECT is a free service offered to our patients.

How do I sign up?

Patients who wish to participate must complete a patient consent form to be issued a Weill Cornell CONNECT activation code during their next visit. This code will enable you to login and create your own user ID and password. You can also download the patient consent form from the login page located at this URL: www.weillcornell.org/connect

Who do I contact if I have further questions?

You should contact your WCMC physician directly.

Who can participate?

All patients who are at least 18 years of age with a participating Weill Cornell provider may participate. In addition, Pediatric patients between the ages of 0-11 can sign up as well. Parents will be given proxy access to manage their children's account.

Is Weill Cornell CONNECT secure?

All communication between you and your healthcare provider and care team using Weill Cornell CONNECT is carried over a secure, encrypted connection. Encryption means the information is coded in such a way that no one can read it during transmission. This secure connection utilizes industry standard Secure Socket Layer (SSL) 128-bit encryption. Your health records are stored behind a firewall to prohibit unauthorized access. In addition, Weill Cornell CONNECT access requires an ID and password that you create yourself.

I forgot my password. How do I get a new one?

From the main login page, "Click on Forgot Password?". After entering your information, you will be prompted with the password question you entered when you first created your account. After correctly answering this question, you will be able to create a new password. If you need additional assistance, please contact your physician office.

I forgot my Weill Cornell CONNECT ID. How do I get a new one?

From the main login page, "Click on Forgot Weill Cornell CONNECT ID?". After entering your information, if you have an external email address on file, your Weill Cornell CONNECT ID will be sent to that email address. If you need additional assistance, or do not have a valid email address on file, please contact your physician office.

When I try to login, I keep receiving the message "Login unsuccessful". What could be wrong?

Make sure your Caps Lock key is not on. The password field is case sensitive. Make sure you do not enter extra spaces in either the Weill Cornell CONNECT ID or password boxes and try again. If you have forgotten your Weill Cornell CONNECT ID or password, click on the "Forgot Password?" link and fill in the form.

Can you send me a new activation code as I have lost it, let it expire or did not receive it?

You can either request a new activation code during your next visit at Weill Cornell, or contact your physician office. Privacy issues prevent us from e-mailing a new activation code to you.

Where can I update my personal information (e.g., home address, e-mail or change my password)?

Log into Weill Cornell CONNECT and from the left menu, go to the Preferences section and select the appropriate option.

When can I see my test results in Weill Cornell CONNECT?

Your test results are released to your Weill Cornell CONNECT account after your physician has reviewed them and released them to Weill Cornell CONNECT.

Why are certain test results not shared electronically via Weill Cornell CONNECT?

Your provider is able to determine which types of test results are able to be accessed through Weill Cornell CONNECT. Further, tests of a very sensitive nature are not released to Weill Cornell CONNECT.

If some of my health information on Weill Cornell CONNECT is not correct, what should I do?

Your Weill Cornell CONNECT information comes directly from your electronic medical record at your doctor's office. Ask your doctor to correct any inaccurate information at your next visit, or send a message to the practice to request changes and updates to your health record. Your health information is reviewed and updated in your electronic medical record each visit.

If I send a message to my doctor or nurse, when can I expect a reply?

You will generally receive an answer within 1-3 business days. Please note that Weill Cornell CONNECT should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.

What is proxy access?

Proxy access is when you access the medical record of another patient in Weill Cornell CONNECT. Examples may include a mother accessing her daughter’s chart, a husband accessing his wife’s chart, or a caregiver accessing the chart of his or her patient. The person accessing the chart is known as the proxy. Proxy access is only available for Pediatric patients up to the age of 11 to a parent or legal guardian. We currently are not giving proxy access for patients who are over 11 years of age.

Can I ask questions regarding a family member from my Weill Cornell CONNECT account?

No, you should not ask questions regarding a family member from your Weill Cornell CONNECT account. Weill Cornell CONNECT offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.

Can my spouse and I share one Weill Cornell CONNECT account?

No, due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own Weill Cornell CONNECT account.

I was logged out of Weill Cornell CONNECT. What happened?

We aim to protect your privacy and security of your information. While logged into Weill Cornell CONNECT, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of Weill Cornell CONNECT. We recommend that you log out of your Weill Cornell CONNECT session if you need to leave your computer for even a short period of time.

What technical requirements are needed to use Weill Cornell CONNECT?

You need access to a computer connected to the Internet and an up-to-date browser (such as Internet Explorer).

My activation code does not work, what should I do?

For your security, your activation code expires after 45 days and is no longer valid after the first time you use it. If you still have problems, contact your physician office.

Is my activation code my user ID?

No, your activation code is not your Weill Cornell CONNECT ID or password. You will use this code only once to log into Weill Cornell CONNECT for the first time. (The code will expire after you have used it or after 45 days). When you log into Weill Cornell CONNECT the first time, you will then be asked to create your own unique Weill Cornell CONNECT ID and password.

I am not receiving email messages notifying me when new information is available in Weill Cornell CONNECT. Why not?

Your email address on file may not be correct. Log into Weill Cornell CONNECT and from the left menu, go to the Preferences section and select the appropriate option to verify your email address.
